Shift: Saturday 8:00 AM – Monday 8:00 AM, Every other weekend

Compensation: $700 flat weekend rate + $28–$33/hour for visit time (DOE)

Summit West Care, a mission-driven non-profit home health agency, is seeking a Weekend On-Call Triage LPN to support patients outside of standard business hours. This role is ideal for an experienced LPN who wants supplemental income, autonomy, and meaningful clinical work - without a full-time schedule.

You ll serve as the calm, knowledgeable clinical resource patients rely on when urgent needs arise after hours.

About The Role

This is a non-traditional, weekend-only position, providing on-call triage coverage and in-person visits when clinically necessary. The role is largely remote, with field visits as needed based on operational and patient needs.

What You ll Do

• Provide on-call triage coverage from Saturday 8:00 AM to Monday 8:00 AM
• Respond to patient calls, assess clinical needs, and determine appropriate next steps
• Complete in-home visits when scheduled or clinically indicated
• Collaborate with RNs, caregivers, and care team members to address urgent patient needs
• Document triage calls and visits accurately and timely
• Work remotely when not completing field visits

Compensation & Key Details

• $700 flat stipend for the full weekend on-call shift
• $28–$33/hour for in-person visit time (based on experience)
• Mileage reimbursement for all work-related travel
• Ability to work from home between visits

What You Bring

• Active Colorado LPN license (required)
• Home health experience preferred, HCHB experience a plus!
• Strong clinical judgment and independent decision-making skills
• Confident communication and triage assessment abilities
• Reliable transportation, valid driver s license, and auto insurance
• Comfort working autonomously in patients  homes
