Pay: $80,000 – $95,000 per year

Schedule: Tuesday – Friday | 10:00 AM – 8:30 PM
Work Location: On the road (travel required)

Vitality Hospice & Palliative Care is seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our clinician-owned and operated hospice agency. • Complete comprehensive hospice admissions and patient/family assessments
• Ensure accurate documentation and appropriate care planning
• Provide direct patient care as needed, including medication administration and treatments
• Collaborate with physicians and the interdisciplinary team to develop individualized plans of care
• Serve as a liaison between patients, families, and healthcare providers
• Educate patients and caregivers on disease progression, symptom management, and hospice services

• Active Registered Nurse (RN) license (required)
• 1+ year of skilled nursing experience preferred
• Hospice or admissions experience preferred (not required)
• Willingness to travel within assigned service areas
• Strong communication, organizational, and interpersonal skills
• Valid driver’s license, reliable transportation, and auto liability insurance
