We are seeking a compassionate Nurse Practitioner (NP) to provide in-home primary care. In this role, you will assess, diagnose, and manage acute and chronic conditions, develop individualized care plans, collaborate with our care team, and educate patients on preventive health.
Key Responsibilities- Perform in-home patient visits
