Visiting Registered Nurse (RN)/Case Manager – Advanced Maintenance Solutions, LLC

Job Title: Visiting Registered Nurse (RN)/Case Manager
Location: Lake County, IN (remote home visits)
Pay: $55.00/hr – $100.00/hr (per-visit rates: $55 per Regular Visit, $100 per Start of Care or Resumption of Care, $75 per Recertification or Discharge visit)

Employment Type: Full, part‑time, and PRN positions available.

• Assess and chart patients’ conditions and needs at each home visit, including medication reviews and vital signs.
• Initiate and update the Plan of Care, revising goals and interventions as patients’ status changes.
• Administer medications and treatments as prescribed by the physician.
• Provide education to patients and family members on proper home health procedures and strategies.
• Coordinate with physicians, physical therapists, occupational therapists, MSWs, and other members of the care team.
• Act as Case Manager when assigned, assuming responsibility to coordinate care for the assigned caseload.
• Perform supervisory visits with home health aides (CNA or HHA) once every two weeks.
• Prepare clinical notes, update the primary physician, and communicate any changes in patients’ conditions.
• Report updates, concerns, and issues to the home office as necessary.

• Graduate of an accredited nursing school.
• Current license in the state of Indiana and current CPR certification.
• Home healthcare experience preferred.
• Excellent observation, verbal and written communication skills, problem‑solving abilities, basic math skills, and nursing competency checklist demonstrated.
• Physically able to walk, stand, lift, transfer patients, and perform prolonged or considerable walking or standing; able to lift supplies and equipment; capable of reaching, stooping, bending, kneeling, or crouching as required.
• Licensed driver with an automobile insured under the state of IN; automobile in good working order.
