Job Summary and Responsibilities

Dignity Health Medical Group - Arizona is seeking a PRN Transitional Care Nurse Practitioner or Physician Assistant to join our team in Phoenix, AZ.

In collaboration with the physician/medical director, assumes clinical responsibility for tasks performed within Nurse Practitioner's or Physician Assistant's scope of practice. Including: taking patient's history, performing physical examinations, ordering appropriate diagnostic tests, establishing diagnosis and provide care and management for common illnesses and disease prevention in the pediatric and adult patient. The APP may assist in the admission, transfer or discharge from Health Care System.

Makes appropriate referrals to specialists and community agencies. Participates in quality assurance and quality improvement program. Works within multidisciplinary team to develop plan for the patient and their family.

The APP will engage in direct patient care activities and medical/clinical practice as the principal component of his/her overall professional responsibility. Patient care activities will include, but are not be limited to:

  • Assessment, evaluation, diagnostic determination, on-going treatment and clinical management, care planning and modification, medications reconciliation, risk assessment, and, coordination of care with the Acute Care Providers, Transitional Team and Primary Care Physicians.
  • Direct contact and consultation with patient family members, advocates, and others to ensure effective communication of patient status and care plans, and to encourage/support an open visitor-friendly environment within the Transitional Clinic.
  • Day-to-day patient care management activity that includes professional input from all members of the patient care team.
  • Accurate and timely completion of all clinical and professional records and documentation to assure coordination of care.
  • Perform other related duties incidental to the work described herein.
  • Job Requirements

    MINIMUM:

    NP or PA Master's Degree. Graduate of accredited program.

    Three years clinical experience in healthcare.

    AZ license.

    Registered in AZ as an NP or PA. Hold and maintain national certification in NP or PA area, under AZ regulations.

    American Heart Association BCLS.

    PREFERRED:

    1+ years of NP/PA experience.

    ABOUT THE POSITION:

    A Transitional Care Nurse Practitioner or Physician Assistant for Dignity Health Medical Group is an Advanced Practice Provider who provides clinical care for high risk/ complex care patients recently discharged from a Dignity health facility. The goal of the APP is to prevent unnecessary hospital readmission by providing improved coordination of care and convenient post-acute care access to medical care. Patients will be seen in clinic Monday - Friday from 8am to 5pm.

    Additionally, all providers within this clinic rotate in on-call requirements. The Department of Transitional Care currently consists of five APPs and one physician and other resource staff across two clinics;
    St. Joseph's Hospital and Medical Center and Chandler Regional Medical Center.
