Remote - Licensed Practical Nurse (LPN) - Massachusetts Licensure Required. Care Harmony is seeking an experienced LPN to support value-based care management initiatives for patients, with a focus on chronic care management. The ideal candidate has at least 3 years of direct patient-facing work experience, thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can have on a patient's healthcare journey.

$45,760.00/yr - $58,000.00/yr

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
• Identify and coordinate community resources with patients that would benefit their care.
• Provide patient education and health literacy on the management of chronic conditions.
• Perform medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills.
• Assist in ensuring timely delivery of services to patients (Home Health, DME, Home Infusion, and other critical needs).
• Resolve patients' questions and create an open dialogue to understand needs.
• Assist with referrals and appointment scheduling.

• Active Massachusetts License (LPN).
• Technical aptitude - Microsoft Office Suite.
• Excellent written and verbal communication skills.
• Epic experience (preferred).
• Additional state licensures (LPN) are a plus.
• Remote requirements:
  Must have active high-speed internet, a dedicated home office or HIPAA-compliant workspace.
• Ability to work remote in a sedentary role; must be able to speak clearly and listen attentively by telephone for extended periods; and take thorough notes in real-time.
