Hospital based palliative care practice is seeking an Experienced Advanced Practitioner to join their caring, professional team.

The Palliative Care Nurse Practitioner provides consultation in palliative care, symptom management and supportive care to meet the needs of the patient and family as requested by the referring provider and primary team.

The role includes direct patient care through consultation, as well as staff education and support.

The advanced practitioner will facilitate clarification of patient and family goals of care as well as access to appropriate support models of care (i.e. hospice, rehabilitation, on-going aggressive treatment) along with the other members of the Palliative Care team.

The advanced practitioner will assist with the diagnosis, treatment and management of acute and chronic health conditions, especially symptoms such as pain, and dyspnea, related to those conditions.
