Job Responsibilities- Conduct comprehensive assessments to identify early signs of CHF exacerbation and enable timely interventions to prevent hospitalizations.
- Develop and implement personalized treatment plans aimed at achieving maximal doses of GDMT, optimizing heart function, and reducing symptoms.
- Carefully titrate medications to reach target doses of GDMT, monitor for side effects, and ensure patient adherence to therapy.
- Educate patients and families on CHF management, emphasizing medication adherence, lifestyle changes, and early symptom recognition.
- Proactively manage symptoms through home-based interventions to prevent acute exacerbations.
- Collaborate with a multidisciplinary team to provide comprehensive, coordinated care in the home setting.
- Conduct regular home visits or telehealth consultations to monitor progress, adjust plans, and provide ongoing support.
- Provide immediate care for acute CHF symptoms at home, reducing need for emergency rooms and hospital admissions.
- Ensure patients have access to necessary resources such as home health services and support groups.
- Participate in research and quality improvement projects focused on home-based CHF management.
- Current NP or PA licensed in the state of Georgia.
- Strong knowledge of GDMT and CHF management guidelines.
- Excellent communication and patient education skills.
- Ability to work collaboratively within a multidisciplinary team.
