• Conduct comprehensive assessments to identify early signs of CHF exacerbation and enable timely interventions to prevent hospitalizations.
• Develop and implement personalized treatment plans aimed at achieving maximal doses of GDMT, optimizing heart function, and reducing symptoms.
• Carefully titrate medications to reach target doses of GDMT, monitor for side effects, and ensure patient adherence to therapy.
• Educate patients and families on CHF management, emphasizing medication adherence, lifestyle changes, and early symptom recognition.
• Proactively manage symptoms through home-based interventions to prevent acute exacerbations.
• Collaborate with a multidisciplinary team to provide comprehensive, coordinated care in the home setting.
• Conduct regular home visits or telehealth consultations to monitor progress, adjust plans, and provide ongoing support.
• Provide immediate care for acute CHF symptoms at home, reducing need for emergency rooms and hospital admissions.
• Ensure patients have access to necessary resources such as home health services and support groups.
• Participate in research and quality improvement projects focused on home-based CHF management.

• Current NP or PA licensed in the state of Georgia.
• Strong knowledge of GDMT and CHF management guidelines.
• Excellent communication and patient education skills.
• Ability to work collaboratively within a multidisciplinary team.
