Remote - Licensed Practical Nurse; LPN - Mass. Licensure - Day Shift

Remote Licensed Practical Nurse (LPN) with Massachusetts licensure. This role focuses on care coordination and value-based care management for chronic care patients. Requires at least 3 years of direct patient-facing experience and the ability to thrive in a fast-paced, remote environment.

$45,760.00/yr - $58,000.00/yr

• Care Harmony's Care Coordinators (LPN) work with providers to deliver value-based care management initiatives for their patients.
• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
• Identify and coordinate community resources for chronic care management patients.
• Provide patient education and health literacy on the management of chronic conditions.
• Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
• Assist in ensuring timely delivery of services to patients, including Home Health, DME, Home Infusion, and other critical needs.
• Resolve patient questions and create an open dialogue to understand needs.
• Assist with/Manage referrals and appointment scheduling.

• Active Massachusetts LPN license.
• At least 3 years of direct patient-facing work experience.
• Technical aptitude with Microsoft Office Suite.
• Excellent written and verbal communication skills.

• Fully remote position—work from home with no commute.
• Consistent full-time schedule, Monday–Friday; no weekends, with rotational on-call about once per year on average.
