Tele-Triage Licensed Practical Nurse (LPN) – Part Time, Remote

Locations: US-KY-LA? ( Louisville ), US-TX-DALLAS, US-IL-CHICAGO, US-TX-SAN ANTONIO, US-GA-ATLANTA. This is a remote, part-time LPN tele-triage role with compact state licensure required.

• Monitor assigned phone lines during service hours to ensure timely response for caller needs
• Triage symptom-based calls and provide recommendations according to the approved triage protocol
• Triage patients by assessing severity of symptoms and refer to the appropriate setting or level of care
• Document in real time all care-related recommendations and notify local operations of triage outcomes
• Initiate contact with acute care settings to coordinate services for clients in the ED/Hospital and coordinate discharge planning; notify operations of triage recommendations
• Coordinate medication orders with the assigned pharmacy and update MAR/Quick Mar for appropriate administration
• Process prior authorizations to ensure timely documentation and submit to the assigned pharmacy for billing
• Complete assessments and reassessments of persons served as assigned
• Develop/update care plans and determine needs, securing physician orders as needed
• Communicate with beneficiaries primarily by phone, text, online chat, or email
• Ensure compliance with state/local/payer requirements through documentation review
• Provide coaching and education related to health and well-being
• Participate in performance improvement activities and maintain ongoing clinical knowledge through internal/external training
• Document actions in Nurse's Notes or Service Delivery Log
• Maintain timesheet log for weekly submission to supervisor
• Notify practices, physicians, and/or managed care clients of encounters with patients as required
• Participate in applicable meetings and committees as requested (e.g., IDT, Safety/Quality committees, treatment plan meetings)

• Degree/certification from an accredited school of Nursing
• At least one year of nursing experience
• Current LPN licensure in the state of practice, in good standing, with an LPN compact license
• Clinical experience with individuals who have intellectual and developmental disabilities, home health, hospice, or related disorders
• One year of home health experience preferred
