Overview
Telehealth Advanced Practice Provider (NP/PA) role at Devoted Health. This is a fully remote position focusing on delivering world class comprehensive care to members via telehealth, with visits predominantly virtual.
Responsibilities- Conduct primarily telehealth video visits to members with the opportunity for a small volume of home based visits (drive to member’s home). In some instances when appropriate and compliant with licensure, you may also provide telehealth visits to members located in other geographies.
- Perform comprehensive assessment visits (CAVs) including diagnosis/disease review, medication review, and assessment for quality of care interventions (STARS/HEDIS) as well as social and home health/DME needs.
- Collaborate with the member’s care team including PCPs, specialists, pharmacy, clinical nursing, social work, and family/caregivers to coordinate care and deliver a collaborative care plan.
- Nurse Practitioner or Physician Assistant with 3 or more years in outpatient clinical practice.
- Active and clear Nurse Practitioner or Physician Assistant license.
- Active BLS at time of hire and maintained during employment at Devoted Medical.
- Experience in primary care, family medicine, geriatrics and/or palliative care.
- Experience performing Medicare annual wellness visits or in-home comprehensive visits with elderly or Medicare patients.
- Experience performing visits over telehealth video platforms.
- Understanding of managed care, including STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps.
Salary Range: $115,000 - $150,000 / year. Our ranges are broad to allow for growth. The actual base salary offered depends on qualifications, experience, skills, education, licenses, and location. Our Total Rewards package includes:
