Job SummaryThe Registered Nurse (RN)/Case Manager plans, organizes, and directs home‑care services emphasizing health education and clinical care. The RN travels to patients’ homes to administer physician‑dictated services while supporting patients to maintain independence and quality of life.
Essential Job Functions / Responsibilities- Assess and chart observations of the patient’s condition and needs at each home visit.
- Complete evaluation tasks, including medication reviews and vital signs.
- Initiate and revise the Plan of Care as patient status and needs change.
- Provide education to patients and family members on proper home‑health procedures and strategies.
- Develop a care plan based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative actions.
- Administer medications and treatments as prescribed by the physician.
- Provide health‑care instructions to the patient as appropriate per assessment and plan of care.
- Identify discharge‑planning needs as part of the care plan and prepare patient and family for discharge.
- Act as Case Manager when assigned by the Director of Nursing, coordinating care for the assigned caseload.
- Coordinate with physicians, PT/PTs, OT, MSW, and other team members in the care plan.
- Prepare clinical notes and update the primary physician when necessary.
- Communicate with the physician and office regarding the patient’s needs, reporting any changes in condition and obtaining physician orders as required.
- Coordinate with community health–related persons to ensure the care plan is executed.
- Report to the home office regarding any updates, concerns, or issues regarding the patient.
- Home‑healthcare experience required.
- Graduate of an accredited school of nursing.
- Current licensure in the state of Indiana and current CPR certification.
- Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills, and nursing skills per competency checklist.
- Physical ability to walk, stand, lift, transfer patients, handle supplies, and perform required nursing tasks (reaching, stooping, bending, kneeling, crouching, visual and hearing acuity).
- Licensed driver with automobile insured under the state of IN and in good working condition.
