Remote Intake Coordinator - Licensed Practical Nurse; LPN Day Shift

This role offers a range of $43,000.00/yr - $58,000.00/yr, based on skills and experience. Care Harmony's Intake Coordinators (LPN) work with providers to deliver value-based care management for patients.

We seek an experienced LPN with at least 3 years of direct patient-facing work, who thrives in a fast-paced environment, is self-motivated, detail-oriented, and committed to impacting patients' healthcare journeys.

You will identify resources and coordinate needs for chronic care management patients and enjoy meeting and assisting new patients daily.

Responsibilities include:

• Conduct health assessments for newly enrolled patients transferring from the Patient Enrollment team.
• Resolve patient questions and foster open dialogue to understand needs.
• Identify and coordinate community resources beneficial for patient care.
• Provide patient education on managing chronic conditions.
• Assist with medication management, adherence, and refills.
• Ensure timely delivery of services like Home Health, DME, and Home Infusion.

Additional requirements:

• Active Compact/Multi-State LPN license.
• Proficiency in Microsoft Office Suite.
• Excellent written and verbal communication skills.

Remote work requirements:

• Active high-speed Wi-Fi connection.
• Home office or HIPAA-compliant workspace.

Preferred additional skills:

• Epic experience.
• Bilingual abilities.
• Single-state license.

Physical requirements include prolonged sitting, clear speaking, attentive listening, and real-time note-taking.

Pay ranges from $21/hr to $28/hr, with opportunities for overtime. Typical shift hours are 8:00 am - 4:30 pm CST or 9:00 am - 5:30 pm CST.
