:
This is not a remote or work‑from‑home position; you must work on‑site at our Savannah branch.
- Develop, plan, implement, analyze, and organize clinical operations for a specific managed location.
- Conduct or delegate patient assessments and re‑assessments, updating care plans in accordance with company, physician, and health facility policies.
- Manage caregiver assignments and overall delivery of care to all patients served by the location.
- Receive case referrals, review patient information, determine home health or hospice needs, and admit patients when criteria are met.
- Assign appropriate clinicians to each case and provide operational guidance during business hours.
- Assist clinicians in establishing therapeutic goals, setting priorities, and developing Patient Care Plans (POC).
- Monitor case documentation for regulatory and payer compliance; ensure timely audits, billing, and Medicare regulations compliance.
- Coordinate communication among team members, attending physicians, and caregivers to ensure appropriate care and outcome planning.
- Collaborate with Branch Director and Finance to set revenue and budget goals.
- Participate in sales and marketing initiatives.
- Supervise all clinical employees assigned to a specific location, overseeing direction, coordination, and evaluation.
- Handle employee corrective actions and discipline issues fairly with Human Resources and operations leadership.
- Interview, hire, train, and develop direct care clinicians; evaluate performance and recommend in‑service education.
- Assess staff education needs based on documentation review and utilize Utilization Review input to deliver regular education.
- Analyze situations, identify problems, and evaluate alternative actions using performance improvement principles.
- Review documentation for case managers and clinical staff regarding care appropriateness and compliance.
- Execute QA/PI activities and collaborate with Utilization Review on performance data and outcome analysis.
- Provide direct patient care only in emergency situations.
- Act as Branch Director in their absence.
- Interpret company policies to ensure compliance with external authorities and internal documentation standards.
Associate degree in Nursing (ADN) required;
Bachelor of Science in Nursing (BSN) strongly preferred.
- Active, unrestricted RN license or Compact State RN license with Georgia authorization.
- Current CPR and/or BLS certification.
- Valid driver’s license, auto insurance, and reliable transportation.
- Minimum of 2 years experience as an RN.
- Home health experience highly desired; experience in hospice, palliative care, or similar settings preferred.
- Management and people leadership experience required.
- Experience with OASIS highly desired; OASIS certification (HCS‑O, COQS, or COS‑C) preferred.
- Experience with Homecare Homebase (HCHB) preferred.
- Foundational knowledge of CMS PDGM preferred.
- Normal Hours of Operation:
Monday‑Friday, 8 a.m.–5 p.m. (ET). - On‑Call Expectation:
Yes, rotating on‑call shift. - Branch Size: 170+ census, 4.0 STAR rating.
- Annual Bonus Eligibility:
Yes, incentive bonus payable quarterly and annually.
