Location: New York
A leading multidisciplinary healthcare team is seeking a New York–licensed Nurse Practitioner (AGNP preferred) for a locum tenens role in Manhattan
. This position focuses on geriatric and complex-care patients
, combining in-home visits and telehealth to deliver high-impact, patient-centered care.
This is an excellent opportunity for NPs who enjoy autonomy, meaningful patient relationships, and working in one of the most dynamic healthcare markets in the country.
What You’ll Do- Provide in-home and virtual care for geriatric patients
- Manage chronic conditions and urgent clinical needs
- Conduct comprehensive assessments and medication reconciliation
- Collaborate with a multidisciplinary care team
- Document visits and support care transitions
- 4 days in-home, 1 day telehealth
- Uber provided for all clinical travel
- 13-week contract with extension potential
- AGNP certification (PMHNP-only or NNP-only not eligible)
- 1+ year experience with geriatric or complex-care patients
- Comfort with home-based and telehealth care
