Senior Doc is expanding our Palliative Care team and is looking for a Nurse Practitioner or Physician Assistant with a passion for caring for seniors and patients with serious or chronic illness. Position Overview:
We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions.
This position supports inpatient palliative care, community‑based care, and home visits, with flexibility to practice across settings based on patient needs and program demand.
Key Responsibilities:
- Provide comprehensive palliative care consultations across inpatient, community, and home-based settings
- Develop and manage individualized care plans emphasizing comfort, symptom control, and patient goals
- Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
- Serve as a clinical leader within the interdisciplinary team
- Educate patients and families on disease progression, treatment options, and goals of care
- Demonstrate strong time management while managing a diverse patient caseload
- Participate in interdisciplinary meetings, case conferences, and care planning
- Travel within the community to conduct home visits as needed
Qualifications:
- Active California Nurse Practitioner or Physician Assistant license
- Palliative Advanced Practice Clinician experience required
- Experience in inpatient palliative care and/or community-based palliative care
- Strong interdisciplinary team leadership skills
- Excellent time management and organizational abilities
- Comfortable with home visits and community driving
- Minimum 1–3+ years of palliative care, hospice, or serious illness care experience
- Strong communication skills with patients, families, and care teams
- Ability to work independently while coordinating care across settings
Preferred / Plus
Qualifications:
- Advanced Certified Hospice and Palliative Nurse (ACHPN)
- Credentialed at San Diego area hospitals
- Prior experience working across multiple care environments

Pay Range for Posted Region
$160,000 - $180,000 USD
