💲 Pay: $25–$33/hour DOE

We are a mission‑driven, non‑profit home health agency serving the Grand Junction community. In this role, you'll provide skilled, one‑on‑one nursing care in patients' homes - supporting independence, dignity, and quality of life where it matters most.

As a valued member of our home health team, you will:

• Provide skilled nursing care to patients throughout Mesa County
• Perform treatments including wound care, medication administration, catheter care, and related services
• Collaborate with RNs and the interdisciplinary care team to monitor patient progress and update care plans
• Support patients with chronic condition management and health education
• Document care accurately and efficiently using Homecare Homebase (HCHB) EMR (training provided)
• Travel locally for patient visits (mileage and drive time reimbursed)

• $25–$35/hour, based on experience
• Mileage and drive-time reimbursement

• Active Colorado LPN license (required)
• IV certification preferred (not required)
• Wound care experience a plus
• Home health experience helpful, but not required - training provided
• Strong clinical judgment, communication, and organizational skills
• Reliable vehicle, valid driver's license, and auto insurance
