Responsibilities- Provide Primary Care treatment in a worksite‑based setting, including chronic condition management, appropriate referrals, sue wellness exams kele and ordering labs and imaging in alignment with primary care practices.
- Deliver episodic care (low‑acuity urgent care).
- Provide extended episodic acute care, including 4–5 visits for an illness or injury of limited chronicity.
- Educate about chronic conditions and co‑manage with outside primary care if we are not the PCP.
- Oversee and perform CLIA‑waived tests, order prescription medications, and oversee dispensing by an RN where allowed by state law.
- Recommend and order immunizations based on USPSTF preventive services guidelines and ACIP.
- Provide health coaching or refer patients into wellness programs and follow up on all referrals.
- Exhibit excellent interpersonal skills with internal and external customers and in group settings.
- Partner with local HR/Benefits teams to improve employee health.
- Graduate of a Certified NP program.
- Meet the Sweat to practice within the designated state(s) with an active, unrestricted NP license and practice independently where allowed.
- Active, unrestricted NP license in Georgia and ability to maintain it.
- Minimum of three or more years of NP experience, working independently in an internal medicine, family practice setting or onsite health center.
- Experience working in an employer‑based health center.
- Chronic disease management experience.
- Passionate about overall health and wellness and patient education.
- High energy and strong customer‑centric focus.
This role is based in an employer‑based health center in Commerce, GA.
