Job Summary
We are looking for a Practice Nurse to join our friendly, forward‑thinking team. We welcome applications from experienced practice nurses and from nurses in community or secondary care who would like to move into general practice, with full training and mentorship provided.
Ideally, you will have experience in venepuncture, wound care, immunisations (including childhood vaccinations), cervical cytology, ECGs and chronic disease reviews (diabetes experience is desirable). Salary will reflect experience, with the upper range offered to nurses already confident across these areas.
If you are new to general practice, we value transferable skills and are particularly keen to hear from nurses with experience in compression wound care, diabetes or minor illness and injury. You will be supported to develop skills in chronic disease management, including asthma, COPD, diabetes, hypertension, cardiovascular, and renal conditions.
We offer genuine flexibility and can accommodate either two part‑time roles or one full‑time (37.5 hours) post.
Friday working is essential
, with flexibility around other days. Extended hours begin at 7 am, and experienced practice nurses undertaking telephone chronic disease reviews may choose family‑friendly split shifts (e.g. working from home 7‑8 am before bringing children to school, then attending the practice later). Trainee practice nurses will work onsite until fully trained.
A golden handshake of up to £2,000 (DOE/pro rata; T&Cs apply) is available.
Main Duties of the Job- Assess, plan, develop, implement and evaluate programmes to promote health and well‑being, and prevent adverse effects on health and well‑being.
- Implement and evaluate individual treatment plans for patients with a known long‑term condition.
- Identify and manage appropriate treatment plans for patients at risk of developing a long‑term condition.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self‑care.
- Deliver opportunistic health promotion using opportunities such as new‑patient medicals.
- Provide information and advice on prescribed or over‑the‑counter medication, including medication regimens, side effects and interactions.
- Assess and care for patients presenting with uncomplicated wounds.
- Support and advise women requesting information relating to family planning needs.
- Support and manage health needs of women presenting for cervical cytology consultations.
- Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health.
- Implement and participate in vaccination and immunisation programmes for both adults and children.
- Advise, support and, where appropriate, administer vaccinations for patients travelling abroad.
- Promote and deliver evidence‑based care for patients presenting with aural conditions.
- Use auditing to improve best practice.
- Assist senior practitioners in providing minor‑surgery sessions.

Pay scheme: £35,100 to £42,000 a year DOE
