• Manage acute and chronic conditions for residents onsite
• Prescribe treatments, order tests, and educate patients/families
• Collaborate closely with physicians and clinical teams
• Document care in a provider-friendly EHR
• Contribute to facility performance improvement and staff education
• Participate in shared call coverage
• Stay current with clinical advancements and best practices

• Active NP or PA license in Missouri and/or Illinois
• National board certification and MSN preferred
• 2+ years of clinical experience, ideally in geriatrics, acute/post-acute care, or rehab
• Tech-savvy and EHR-proficient
• Strong interpersonal and communication skills
• Team-oriented with a collaborative mindset
