Job Summary
Diabetes Nurse – GP surgery at NHS Stowhealth, Stowmarket, Suffolk. Part‑time (two days per week), one fixed day (Wednesday) and one day chosen by the applicant with the option to work from home.
Responsibilities- Run a Long Term Conditions clinic on Wednesdays for adults with type 2 diabetes.
- Initiate and titrate GLP‑1 therapies and other injectable diabetes treatments.
- Support the Senior Pharmacist with poorly controlled diabetes patients.
- Respond to non‑urgent diabetes‑related patient queries.
- Provide clinical support to the Physician Associates delivering the newly diagnosed diabetes service.
- Upskill the existing practice nursing team and provide training and supervision.
- Registered Nurse (adult care) with current NMC registration.
- Post‑registration qualification in diabetes care (diploma, degree module or equivalent).
- Evidence of ongoing CPD in diabetes and long‑term conditions.
- Desirable:
Independent Nurse Prescriber or willingness to work towards this.
- Significant post‑registration experience in diabetes care in primary, community or secondary care.
- Demonstrable experience managing complex type 2 diabetes, including patients with multiple comorbidities.
- Hands‑on experience initiating, titrating and monitoring GLP‑1 receptor agonists and other injectable therapies within protocols and under appropriate clinical supervision.
- Experience working effectively within a multidisciplinary team and delivering patient education and staff training.
- Desirable:
Experience in service development, audit and quality improvement.
- In‑depth knowledge of type 2 diabetes pathophysiology and management, national guidance (NICE, local pathways) and GLP‑1 therapy requirements.
- Strong clinical assessment and decision‑making skills, with the ability to manage complex cases within agreed protocols.
- Excellent communication, consultation and motivational interviewing skills.
- Ability to teach and support patients in self‑management, injection technique and lifestyle modification.
- Competence in using clinical IT systems, data extraction tools and recall systems.
- Understanding of health inequalities and the impact of social determinants on diabetes outcomes.
- Compassionate, patient‑centred approach.
- Ability to work autonomously within scope of practice and as part of an MDT.
- Organised, with good time‑management and prioritisation skills.
- Commitment to continuous improvement and evidence‑based practice.
