Base Pay RangeWoundology Clinical Coordinators (LPN) work comprehensively with providers to deliver wound care management for their patients. Woundology is seeking an experienced Licensed Practical Nurse to work 100% Remote – LPN Nurse (LPN) with at least 2 years of direct patient‑facing work experience; that thrives in a fast‑paced environment, is self‑motivated, has impeccable attention to detail, and values the impact they can have on a patient’s healthcare journey.
Our home care LPNs will use cutting edge technology and clinical knowledge to provide exceptional care to patients who require one‑to‑one care and attention in their homes.
- Fully remote position with flexible hours – Remotely in home and individual patient homes
- Consistent schedule – Full‑Time Monday – Friday, no weekends
- Manage patient census with a resolution‑driven approach to close gaps in clinical and non‑clinical patient care.
- Identify and coordinate visits for their wound care interventions weekly.
- Provide patient education.
- Perform Ultramist Therapy for individual patients in their homes.
- Assist in ensuring timely delivery of services to your patients.
- Resolve patients' questions and create an open dialogue to understand needs and procedures.
- Assist/Manage referrals and appointment scheduling.
- Assist with sending documentation to home healthcare teams.
- Active State License (LPN) (NLC) (LVN);
Multistate license preferred. - Basic Life Support – active.
- Technical aptitude – Microsoft Office Suite and EMR systems.
- Wound Care experience required.
- Excellent written and verbal communication skill.
- Must have active high‑speed Wi‑Fi.
- Must have a HIPAA‑compliant workspace.
- Must have drivers license and own vehicle for travel to patients' homes.
- This position will be performing wound care and Ultramist Therapy procedures in the home, so being able to stand and sit or bend is a MUST.
- This position will require the ability to speak clearly and listen attentively, often by telephone and in person.
- The position will require the ability to understand, process, and document procedures each day after completion for providers to cosign.
- Standing Frequently, Walking Frequently, Lifting Frequently up to 50 lbs, Carrying Frequently up to 50 lbs, Balancing Occasionally, Stooping Frequently, Kneeling Frequently.
