As a Staff Nurse
, you will deliver compassionate, professional nursing care directly at patients’ homes—supporting their physical comfort, emotional well-being, and overall quality of life.
You may be assigned to either Hospice Care or Home Care
, depending on experience and service needs.
- Conduct regular home visits and nursing assessments.
- Develop, implement, and review individualised care plans.
- Administer medications and perform nursing procedures (e.g. wound care, catheterisation, NG tube).
- Educate and support caregivers in home-based care and symptom management.
- Coordinate referrals and collaborate with doctors, therapists, and community partners.
- Maintain accurate documentation in line with PDPA and service standards.
- Support occasional outreach, volunteer, or fundraising activities.
- Diploma/Degree in Nursing, registered with Singapore Nursing Board (SNB).
- At least 2–3 years of clinical experience (community, palliative, or home nursing preferred).
- Palliative Care certification is an advantage.
- Empathetic, self-motivated, and able to work independently.
- Comfortable with home visits and community-based care.
