NURSE I - Veterans Community Living Center at Rifle

OverviewThe Colorado State Veterans Community Living Center at Rifle, located on the western slope of Colorado, provides 89 beds of skilled nursing care, including an 18‑bed Special Care Unit for residents with Alzheimer’s Disease or related conditions. This position honors and respects residents through companionship, empowerment, and individualized care.
PrimaryJob Duties
- Performs admission and daily assessments during clinical stay, identifying critical incidents, emergencies, acute illnesses, and behavior changes.
- Identifies and implements appropriate nursing interventions, treatment care plans, and physician referrals.
- Documents accurate, current, and pertinent information regarding care and treatment provided to individual residents.
- Indicates specific changes in status, medications, and physician orders.
- Detects abnormality in resident’s health status indicated by symptoms, vital signs, and laboratory results and reports information to physician.
- Required to drive a State vehicle for off‑site pre‑admission assessments or transporting residents to external care.
- Directs and implements nursing care, ensuring compliance with accepted standards of nursing care, departmental and facility policies, and VA and State of Colorado regulatory requirements.
- Directs subordinates to assure these standards are met.
- Reviews and audits physician orders, MARS, CNA flow sheets, bathing schedules, and BM sheets to determine compliance with policies and standards of care.
- Performs prescribed, specialized treatments and ensures safe and accurate medication administration.
- Responsible for IV administration & venipuncture according to physician orders, State, Federal, and VA regulations.
- Monitors the clinical, physical and environmental safety of all residents through assessments and regularly scheduled rounds, and notifies family, physician, NHA, and DON as appropriate.
- Serves as a resource to subordinate staff to assure timely and proper care of residents.
- Trains subordinate staff and ensures competencies are completed and submitted.
- Provides input to management on the direct care performance, documentation, attendance and interpersonal skills of subordinates for evaluation purposes.
- Evaluates daily workload and prioritizes assignments with available staff, delegating duties to ensure quality and continuity of care.
- Performs other duties as assigned, maintaining updated skills, knowledge, and competencies through continuing education and team development.
- On‑call may be necessary on special occasions.
- Provides active listening and communication, support and encouragement to residents and families via phone and personal contact.
- Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC).
- No substitutions allowed.
- Current and active RN license renewal required.
- Willing to undergo a full background check.
- Position requires travel to conferences and industry‑specific trainings/meetings.
- Must possess a valid driver’s license and be willing to undertake a TB test.
- Shift work: day, evening, and night shifts, including holidays and weekends as needed.
- On‑call rotation as defined in the facility protocol.
