This entry-level Field Registered Nurse position is designed to develop fundamental nursing skills within a home health setting and serves as a foundational role for nurses aspiring to advance into case management. This position works under a Case Manager Registered Nurse.
Anticipated Coverage Area:
Davis, CA & Surrounding Cities.
- As Home Health Field Nurse you will provide hands-on patient care within RN license scope, including procedures such as PICC line care, wound care, feeding pumps, NG/G/J tube management, drain management, catheter care, trach care, blood draws, ostomy management, injections, IV antibiotics/TPN administration, and related tasks as applicable.
- Provide direct patient care as defined by the state Nurse Practice Act.
- Implement a plan of care initiated and directed by the Case Management Registered Nurse.
- Provide accurate and timely documentation within 24-48 hours consistent with the plan of care.
- Educate patients and family/caregivers regarding diagnosis and plan of care.
- Assist the RN with skilled procedures and prepare equipment and materials for treatments.
- Coordinate home health services and communicate identified needs to the Case Management RN or other disciplines as appropriate.
- Use equipment and supplies effectively and efficiently.
- Participate in personal and professional growth and development.
- Perform other duties as assigned by the Case Management Registered Nurse.
- Record communications and prioritize care; provide timely reports to the Case Manager and provider.
- Maintain patient privacy and confidentiality in medical records and care delivery.
- Maintain professionalism and enhance clinical skills; adhere to productivity standards as directed by the Case Management RN.
- Complete and review Start of Care assessments, documentation, and plans of care with the Case Manager as needed.
- Participate in on-call duties as required by weekend rotation.
- One (1) year of recent nursing experience; experience requirement may be waived for internal candidates and/or new grad postings.
- One year of home health care experience is preferred.
- Experience with high-acuity patients is a plus; experience working with an interdisciplinary team is highly desired.
- Graduate of an accredited school of nursing (Bachelor's degree).
- Current CA Registered Nurse License (RN).
- Current Basic Life Support (BLS) Card.
- Valid CA Driver's License with acceptable driving record.
- All licenses and certifications must be current at time of hire and sustained throughout employment.
- Self-directed, critical thinker with excellent interpersonal, organizational, problem-solving, and communication skills; basic math skills; nursing skills per competency checklist.
- Must maintain current compliance training and certifications.
