Position Summary
We are seeking a dedicated and reliable Licensed Vocational Nurse (LVN) to join our skilled nursing team. As an LVN in home health, you will provide direct patient care under the supervision of a Registered Nurse and in accordance with the plan of care. Visits may include wound care, medication administration, chronic disease monitoring, and patient/family education.
Responsibilities
Perform skilled nursing visits to patients in their homes in accordance with the care plan
Accurately document assessments, care provided, and patient response in EMR (Synergy experience preferred)
Monitor and report changes in patient conditions to supervising RN and physician
Provide wound care, catheter care, injections, and medication management
Collaborate with interdisciplinary team to ensure coordinated, patient-centered care
Educate patients and caregivers to promote independence and safety
Comply with Medicare/Medicaid and state regulations
Qualifications
Current LVN license in the State of Nevada (unrestricted)
One (1) year of clinical experience (home health preferred)
Valid driver’s license, auto insurance, and reliable transportation
CPR Certification (BLS)
Ability to work independently and manage time effectively
Proficient in electronic documentation (EMR experience preferred)
This is a remote position.
