• Conduct assessments
• Assist in development of care plans and coordination of care
• Assist in implementing physician orders and follow-up, medication refills, patient/family education, and identification of opportunities for preventative screening; notify member test results
• Coordinate with front office team to schedule appointments and meetings where applicable
• Communicate effectively with providers and all departments to resolve issues
• Prepare members for procedures and exams and obtain vital signs
• Utilize EMR (Electronic Medical Records)
• Maintain supplies in the exam room

• Graduate of an approved LPN/LVN program
• Current licensure as an LPN/LVN in the applicable state
• At least one year of experience in a clinic or physician's office
• Current BCLS required
• Phlebotomy skills
• Strong computer skills including electronic health records
• Excellent customer service skills; team oriented; strong time management
• Responsible and reliable; ability to work health center hours

• At least 1 year of front and back-office experience
• Customer-centric focus
• Dynamic personality
• Additional languages helpful but not required

This full-time role is based onsite in our health center in Thomasville, NC.

If you will be working from home occasionally or permanently, internet should be through cable broadband or fiber optic with speeds of at least 10 Mbps download/5 Mbps upload.
