Remote - Licensed Practical Nurse - LPN - LVN - Intake Coordinator
This Care Harmony role is for an experienced Licensed Practical Nurse (LPN/NLC/LVN) with at least 3 years of direct patient-facing work experience who thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can have on a patient s healthcare journey. The Intake Coordinator works comprehensively with providers to deliver value-based care management initiatives for their patients.
You will have experience identifying resources and coordinating needs for chronic care management patients. This role is great for anyone who loves meeting and assisting new patients daily.
Base pay range: $43,000.00/yr - $58,000.00/yr

- Resolve patients questions and create an open dialogue to understand needs.
- Identify and coordinate community resources with patients that would benefit their care.
- Provide patient education and health literacy on the management of chronic conditions.
- Assist with medication management, including identifying potential medication concerns, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services to patients;
Home Health, DME, Home Infusion, and other critical needs. - Adapt quickly to a fast-paced work environment; most of the shift is on the phone.
- Active Compact/Multi-State license (LPN) (LVN).
- Technical aptitude - Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi.
