Job DescriptionFull‑time LVN position, 30 productivity points per week with opportunity for overtime pay. Responsibilities include comprehensive care delivery, skilled documentation via Homecare Homebase, wound care, medication administration, teaching, and care coordination.
Position Requirements- Comfortable treating wounds
- Advocacy and ongoing communication with physicians, referrals, caregivers, and facility management
- Administer medications and treatments as prescribed; provide direct patient care including infusion therapy, wound care, teaching, disease management
- Participate in regular case conferences and in‑services
- Service approximately 5 patients a day, Monday–Friday
- Strong organization skills, time management, multitasking, and adherence to timelines
- Professional appearance and effective communication skills
- Current Texas LVN license
- Three years of clinical nursing experience
- Current Texas driver's license
- CPR certification required
- Experience with healthcare software
- Competency to perform any skilled procedure within scope of the Nurse Practice Act as ordered by a physician
