Hybrid Divisional RN Care/Case Manager
Join Center Well Senior Primary Care as a Hybrid Divisional RN Care/Case Manager. The role combines in‑clinic and remote responsibilities with a focus on member wellness.
Pay RangeBase pay: $71,100 - $97,800 per year. Pay varies by skill and experience.
Responsibilities- The RN assesses and evaluates members’ needs and requirements to help them achieve and maintain optimal wellness by guiding members/families toward appropriate resources and facilitating interactions.
- The RN undertakes varied work assignments that frequently require interpretation and independent determination of appropriate courses of action.
- The RN employs a range of strategies and techniques to manage physical, environmental, and psychosocial health issues, identifies and resolves barriers to effective care, and ensures patients progress toward desired outcomes by continuously monitoring and evaluating patient care.
- The RN may create member care plans, and understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
- The RN makes decisions about work methods with minimal direction, following established guidelines and procedures.
- Registered Nurse (RN license)
- Minimum of 4 years of experience working in health care services and navigating community‑based resources
- Registered Nurse with 2 years of experience in case/care management
- Must live and reside in the Las Vegas metro area
- BSN
- 3-5 years of assessment and care coordination experience
- Experience working with the adult population
- Previous managed care experience
- Bilingual — English, Spanish
- Certification in Case Management
- Motivational Interviewing Certification and/or knowledge
- Knowledge of community health and social service agencies and additional community resources
- Exceptional communication and interpersonal skills with the ability to quickly build rapport
- Ability to work with minimal supervision within the role and scope
- Ability to use a variety of electronic information applications/software programs including electronic medical records
- Intermediate to Advanced computer skills, Microsoft Word, Outlook, Excel
- Excellent keyboard and web navigation skills
- Ability to work a full‑time (40 hours minimum) flexible work schedule
- Must have a separate room with a locked door that can be used as a home office, ensuring continuous privacy while working
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences
- Experience with health promotion, coaching and wellness
This role has a mobile presence and may require regular onsite engagement with the care team at assigned clinics to see patients in person and collaborate with care team members.
Workstyle and LocationHybrid – combination of in‑clinic and remote work.
Must reside in the Las Vegas Metro area.
Hours40‑hour work week, Monday through Friday, 8:00 AM to 5:00 PM. Incremental time may be requested to meet business needs.
TB ScreeningThis member‑facing role is part of Humana’s Tuberculosis (TB) screening program. Applicants will be required to be screened for TB.
Driver’s License, Reliable Transportation, Insurance- Valid state driver’s license
- Insurance in accordance with state minimum limits or $25,000/$25,000/10,000, whichever is higher, and a reliable vehicle
