Overview
Nurse Practitioner – Geriatric Primary Care – Residency Opportunity at Fraser Health Authority in Langley, BC. This role focuses on geriatric home-based primary care for frail, home-bound older adults. We are seeking a dedicated Nurse Practitioner (NP) to contribute to Geriatric Home-Based Primary Care.
Responsibilities- Collaborate with the Langley Seniors Clinic and other community health services to deliver comprehensive medical care to older adults with complex health needs.
- Develop and manage individualized care plans focused on chronic disease management, medication optimization, and preventive health.
- Work within a multidisciplinary team to support safe, effective, and timely care transitions.
- Advocate for patients and families, helping them navigate health systems and access appropriate resources.
- Contribute to quality improvement initiatives and the advancement of best practices in geriatric care.

Be part of our clinical enhancement program with regular in-house skills development such as 12 lead ECG interpretation, Point of Care ultrasound (POCUS), suturing and x-ray interpretation, plus centralized NP orientation. You will be welcomed into a New Graduate NP Community of Practice to support your transition into independent practice.

Education and Experience
Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM). Recent relevant clinical nursing experience.
