Responsibilities
  • Primarily support in-home, patient-facing visits to meet patient care needs in the market.
  • Perform primary care in-home services, initial patient screenings/physical examinations, develop appropriate assessment/plan, and complete documentation in a timely manner.
  • Collaborate with other clinicians in the Care Community to manage the health and well-being of patients living in their homes.
  • Engage attributed patients into the Well Be program and support care needs and efforts to keep patients engaged.
  • Provide educational resources to aid staff, patients, and families in decision-making.
  • Address urgent care needs as needed, including transition of care visits.
  • Collaborate with the community team to complete full patient care assessments and plans.
  • Order, interpret, and analyze diagnostic tests and determine follow-up or further treatment options per state supervisory agreement.
  • Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents per state supervisory agreement.
  • Update patient records and verify accuracy at each appointment.
  • Deliver patient care and conduct regular patient reviews with supervising physician per state supervisory agreement.
  • Focus on documentation, coding, HEDIS measures, patient engagement, and managing medical expenses.
  • Utilize available resources to meet patient needs and refer to internal/external resources as appropriate.
  • Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
  • Other tasks as needed to accomplish team objectives.
Qualifications
  • Master’s Degree in Nursing with a focus on Family Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner, or Adult-Geriatric Nurse Practitioner from an accredited program (required).
  • Active, unencumbered NP license in the state of the job posting and residency in that state (required).
  • Current board certification (ANCC or AANP) (required).
  • BLS certification and an active NP NPI number (required).
  • CPR certification (required).
  • Active, unencumbered DEA licensure or eligibility to obtain (required).
  • Valid driver’s license and access to a car; willingness to drive to patient homes (required).
  • Proof of active auto insurance policy (required).
Skills We Value
  • 3+ years of Nurse Practitioner experience (preferred); new grads may be considered.
  • Experience in related healthcare roles (preferred).
  • Experience in value-based care, home care, primary care, hospice/palliative care, or geriatrics (preferred).
  • Experience with telehealth care delivery (preferred).
  • Ability to conduct in-home visits in various residential settings and on a telehealth platform.
  • Proficiency with Microsoft Office (Excel, Word) and general office software including Electronic Health Records and scheduling tools.
Supervisory and Travel

This position will have supervisory responsibility.

Travel may be required up to 100% locally to deliver patient care.

Work Conditions

Ability to lift up to 30 lbs; assistance may be required for lifting up to 50 lbs and transferring patients up to 100 lbs. Ability to stand for extended periods and drive to patient locations. Fine motor skills and visual acuity required.
