We are seeking a dedicated and compassionate Mobile Wound Care Licensed Practical Nurse (LPN) to join our growing team. The ideal candidate will provide hands‑on wound care services, assist in patient education, and collaborate with healthcare professionals to ensure optimal healing outcomes. This role requires strong clinical skills, attention to detail, and the ability to work independently in a mobile healthcare setting.
Key Responsibilities- Provide direct wound care treatment under the supervision of a Nurse Practitioner (NP) or Physician.
- Perform wound assessments, dressing changes, debridement assistance, and infection control measures.
- Assist with advanced wound care techniques, such as negative pressure wound therapy and compression therapy.
- Educate patients, caregivers, and facility staff on wound prevention and treatment strategies.
- Document patient encounters in an EMR system in compliance with HIPAA and healthcare regulations.
- Monitor patient progress and report significant changes to the supervising provider.
- Coordinate with home health agencies, physicians, and other healthcare professionals to ensure continuity of care.
- Travel to patients’ homes, skilled nursing facilities, and long‑term care settings within the assigned service area.
- Current LPN license in Nevada.
- Minimum of 1 year of experience in wound care, home health, long‑term care, or a related field.
- Wound Care Certification (WCC) or similar is preferred but not required.
- Strong ability to work independently and autonomously in the field.
- Excellent communication and documentation skills.
- Must have a valid driver’s license and reliable transportation for travel within the service area.
