Role Description

This is a full-time hybrid role for a Registered Nurse at Buckeye Hospice and Palliative Care Services, located in Medina, OH, with some work-from-home flexibility. The Registered Nurse will be responsible for providing direct patient care, managing patient assessments, creating and updating care plans, and coordinating with interdisciplinary teams. The role also involves educating patients and their families on care plans and providing emotional support to both patients and their families.

Qualifications
  • Active Registered Nurse (RN) license in the State of Ohio
  • Previous experience in hospice and palliative care
  • Ability to perform patient assessments and create individualized care plans
  • Strong communication and interpersonal skills
  • Capability to work independently and as part of a team
  • Proficiency in using electronic health record (EHR) systems
  • Compassionate, empathetic, and patient-centered approach to care
  • Valid driver's license and reliable transportation
  • Certified in CPR and Basic Life Support (BLS)
