• Develop positive relationships with assigned individuals and families
• Assess client needs and facilitate appropriate responses
• Monitor cases by verifying clients' attendance and evaluating treatments and responses
• Communicate client progress through weekly meetings and evaluations
• Collaborate with other organizations to provide comprehensive support to clients

• Bachelor's degree in a human services area or equivalent experience in case management services
• Experience providing case management and community support services, particularly to individuals with substance abuse or co-occurring disorders
• Valid driver's license and willingness to travel locally up to 30% of the time
• Basic computer skills and access to reliable internet service during working hours
• Ability to pass a reference check, background check, and client drug screen

• Experience working with corrections-involved clients/offenders
• Demonstrated passion for customer/partner responsibility and community service
• Strong self-starting and hardworking attitude with a commitment to innovation
• Proven leadership skills and a relentless commitment to achieving goals
