Overview
As part of the Therapy Hub, you will be responsible for maximising the independence and autonomy of people, ensuring that interventions are provided fairly and in a manner that is sensitive to individual needs. You will have good assessment and treatment planning skills, prioritise workload and work within a multi-disciplinary rehabilitation approach. You will work in partnership with the Early Intervention Community Team to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.
Responsibilities- Ensure access to urgent and routine Occupational Therapy assessment and intervention.
- Provide and monitor treatment programmes at home and ensure safe discharge or referral to appropriate services.
- Work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual plans of care relevant to patient need.
- Work in a community setting requiring reliable transport or means of travel.
- Provide professional advice and appropriate training to others in the multi-disciplinary service and contribute to service development.
- Enjoy a comprehensive supervision, training and development programme, including a Preceptorship for newly qualified staff and mentorship for new recruits.
- Work multi-professionally and support other team members to meet service goals.
