The OnTrack

NY Program is a Coordinated Specialty Care (CSC) treatment program at Community Health and Behavioral Services serving adolescents and young adults who are impacted by unexpected changes in their thinking and perceptions. NY team staff receive expert training and technical assistance in the CSC model and protected time for professional development through the statewide OnTrack

NY Initiative at the Center for Practice Innovations at Columbia Psychiatry and New York State Research Foundation for Mental Hygiene.

Core Responsibilities

The Primary Clinician and Outreach and Recruitment Coordinator (PC/ORC) is a full‑time position. Serves as the Primary Clinician for a subset of program participants and supports with Outreach and Recruitment duties.

Primary Clinician (PC) Role, the duties include:
  • Serve as the point person for the participant and their family.
  • Provide services in the community (off‑site) as needed, based on participant and family preferences.
  • Conduct needs assessments.
  • Engage in safety planning and wellness management planning with participants.
  • Connect participants and families with the services and supports they need.
  • Provide behavioral interventions and take an especially active role in the family support domain.
  • Conduct clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision‑making framework.
  • Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho‑education.
  • Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrack

    NY program.
  • Provide supportive therapy.
  • Develop and modify treatment using a shared decision‑making process with clients.
  • Work with participants and family for transition from the team to other services and supports and ensure stable links at discharge.
  • Rotate on‑call responsibilities with other team members.
  • Attend weekly team meetings.
Outreach and Recruitment Coordinator (ORC) duties include:
  • Establish warm and collaborative relationships with community partners who might refer or provide ongoing support to young people and families.
  • Spread awareness about OnTrack

    NY services through community education and outreach activities that emphasize OnTrack

    NY’s uniquely flexible and youth‑friendly approach.
  • Collaborate with OnTrack

    NY team members to set new community‑based goals focused on reducing inequities and barriers to accessing specialty care.
  • Identify and create new resources to support diverse young people and families seeking or receiving services at OnTrack

    NY.
  • Ensure a welcoming and efficient process for young people and families seeking care in OnTrack

    NY.
  • Serve as primary contact person for referral sources and maintain ongoing communication with referral sources.
  • Conduct telephone and in‑person eligibility screening of young people referred to OnTrack

    NY.
  • Refer participants ineligible for services to other appropriate community providers.
  • Establish and maintain a database of referral sources with up‑to‑date contact information.
Required Qualifications
  • Licensed Masters or Ph.D. in an appropriate field (social work, psychology, mental health counseling).
  • Valid NYS Driver’s License.
Preferred Qualifications
  • Training and experience working with adolescents and/or young adults with serious mental illness.
  • Experience in community‑based mental health services.
