Per Visit Rates:
$75 per regular ST Visit
$112.50 per ST Eval

Job Description:The Speech-Language Pathologist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
- Evaluate patients and perform appropriate therapy interventions
- Develop a comprehensive plan of care and coordinate multi-disciplinary care
- Perform clinical documentation and visit notes
- Continued communication with physician, care team members, patient and family
- Comfortable working in all aspects of the home health setting
- Implement and maintain SLP protocols
- Manage patient treatment and outcomes directly
- Current Texas Speech-Language Pathology License
- One year of SLP experience
- Experience with Homecare Homebase is helpful, but not required
- Previous Home Health experience is helpful, but not required
- Current Texas driver's license
- CPR certification
