Our client, a home health care provider serving patients across southwestern Connecticut, is seeking a compassionate and motivated Physical Therapist to deliver high-quality rehabilitative care in the home setting. • Provide skilled physical therapy services to patients in their homes.
• Perform comprehensive evaluations of patients’ physical conditions and functional abilities.
• Develop and implement individualized treatment plans tailored to patient goals and needs.
• Educate patients and family members on therapeutic exercises, recovery strategies, and lifestyle modifications.
• Monitor patient progress and adjust treatment plans to ensure optimal outcomes.
• Collaborate with other healthcare professionals to support coordinated, comprehensive patient care.
• Maintain appropriate clinical documentation in accordance with professional and regulatory standards.

• Graduation from an accredited Physical Therapy program.
• Active Physical Therapist licensure in the state of Connecticut.
• Minimum of one (1) year of experience as a practicing Physical Therapist.
• Strong interpersonal, communication, and patient-care skills.
• Ability to work independently and manage time effectively.
• Demonstrated commitment to compassionate, patient-centered care.

• Prior experience providing physical therapy services in a home health setting.
• Experience working with diverse patient populations in community-based care.
