Starting annual salary $48,300 commensurate upon experience
Hybrid position!
Provides intensive home based services to children at risk for psychiatric hospitalization. Services provided include individual and family therapy, parenting skills training, around the clock crisis support, extensive case management, and advocacy. IICAPS operates from a team approach and consists of a Clinician and Mental Health Counselor, both of whom participate in sessions and treatment meetings. Participates in implementation of treatment plan, modifying as appropriate during the course of service.
Services provided are in collaboration with DCF, schools, and other community based providers. Team is responsible for attending Rounds and group supervision weekly and maintaining clinical documentation according to licensing, accreditation, Medicaid, and Yale requirements. Purposes of services are to stabilize the child in the home and transition to traditional outpatient services, improve family functioning, and connect the child with the community in a positive way.
There is expected consultation and coordination care with Psychiatric providers if appropriate. Must maintain productivity targets determined by Service and remain current on all model training requirements.
JOB SUMMARY
The Family Support Specialist is responsible for provision of case management, support services, mental health services to children and families, or community programs, in coordination with other agency and community services and documentation of services as required by program policies and funding sources. Provides service in the office, home or host setting, to meet the unique needs of families served.
KEY RESPONSIBILITIES
- Conducts comprehensive assessments and engagement
- Develops treatment plans and contracts for services
- Implements treatment plans, modifying it as appropriate during the course of treatment
- Discharge planning, which may include plans for follow up treatment and connection to community services and programs
- Provides case management services
- Works with community groups in collaborative ventures
EDUCATION
BA in social work, sociology, psychology or comparable human service program.
EXPERIENCE
- 2-3 years’ experience in a comparable position in a human services agency preferred but not required.
- Excellent oral and written skills and ability to communicate comfortably with children, parents, and school personnel.
