ResponsibilitiesThe role involves providing direct care and individualized support to individuals with complex medical needs. Responsibilities include assisting with activities of daily living such as personal care, meal preparation, and mobility support; implementing care plans and adhering to established medical protocols; and fostering a safe, respectful, and empowering environment. The position also requires promoting community engagement through outings and activities, accurately documenting and reporting client progress, concerns, and incidents, and collaborating effectively with healthcare professionals, family members, and interdisciplinary team members to ensure comprehensive and high-quality care.
Required Qualifications , Skills, and Training- High school diploma or equivalent
- Experience working with individuals with medical complexities, disabilities, or special needs is highly valued.
- Ability to assist with personal care tasks and follow medical care plans.
- Strong communication, patience, and problem-solving skills
- Valid driver’s license and reliable transportation.
- Ability to pass a background check and meet all state requirements.
