Our client, a growing home health agency, is seeking a dedicated and skilled Physical Therapist to support patients in the home setting. This role offers schedule flexibility and the opportunity to work independently while delivering high‑quality, patient‑centered rehabilitative care. The Physical Therapist will play a critical role in improving patient mobility, functional independence, and overall quality of life following illness, injury, or surgery.
Key Responsibilities- Conduct comprehensive physical assessments and review medical histories to evaluate patient needs.
- Develop and implement individualized treatment plans aligned with patient goals.
- Deliver therapeutic exercises, mobility training, and rehabilitation interventions in the home environment.
- Educate patients, families, and caregivers on recovery strategies, injury prevention, and wellness.
- Monitor, evaluate, and document patient progress toward rehabilitation goals, adjusting treatment plans as needed.
- Collaborate with other healthcare professionals to ensure coordinated, holistic patient care.
- Maintain accurate and timely clinical documentation in compliance with regulatory and professional standards.
- Graduation from an accredited Physical Therapy program.
- Master’s degree in Physical Therapy.
- Minimum of one (1) year of experience as a Physical Therapist in a home health setting.
- Active state licensure to practice as a Physical Therapist in the state of operation.
- Current CPR certification (must be maintained while in a clinical role).
- Valid driver’s license with the ability to travel to patient homes within the service area.
- Strong interpersonal skills with a compassionate, patient‑focused approach.
- Solid understanding of physical therapy practices and the ability to adapt treatment plans to the home environment.
- Experience working with diverse patient populations in community-based or home care settings.
- Ability to work independently while managing a flexible schedule.
