Join our dynamic team as a licensed Occupational Therapist (OT) in the scenic Grand Junction area, where you will provide exceptional in-home occupational therapy services to patients eager for your expertise.

• Deliver patient-centered, evidence-based occupational therapy in the comfort of patients' homes.
• Perform a variety of visits including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
• Develop and implement individualized occupational therapy plans tailored to each patient’s needs and goals.
• Offer skilled interventions such as:
• Activities of Daily Living (ADL) training
• Instrumental Activities of Daily Living (IADL) training
• Upper extremity strengthening
• Fine motor coordination
• Adaptive equipment training
• Energy conservation techniques
• Home safety and fall-prevention strategies
• Educate patients and caregivers on their treatment plans and empower them with adaptive techniques and safety strategies.
• Communicate patient progress and concerns to the interdisciplinary care team for seamless coordination.
• Collaborate in the clinical oversight of Occupational Therapy Assistants (OTAs), where applicable, following Colorado law and agency policy.

• Complete all necessary documentation accurately and in a timely manner using Homecare Homebase (HCHB).
• Maintain compliance with Medicare Conditions of Participation and agency standards.
• Uphold HIPAA regulations and ensure privacy and security for all patient information.

• Work collaboratively with clinical staff members, including RNs, OTAs, PTs, STs, and leadership, to coordinate patient care.
• Ensure effective communication related to patient needs and regulatory requirements.

• Current, unrestricted Colorado Occupational Therapist license.
• Graduation from an accredited Occupational Therapy program.
• Valid driver’s license and reliable transportation.
• Ability to practice independently in a home-based clinical setting.
• Willingness to learn or proficiency with Homecare Homebase (HCHB).

• Home health experience.
• Familiarity with Medicare documentation and home health regulations.
• Experience working with geriatric and medically complex patient populations.

• Engage in patient-centered, in-home care with one-on-one visits.
• Supportive clinical and administrative coordination.
• Mission-driven services dedicated to the Grand Junction community.

• Monday - Friday, 8 AM - 5 PM.
