Position: Shared Living Coordinator

Woodfords Family Services is seeking a dedicated, relationship-driven professional to join our team in Manchester, ME as a Shared Living Coordinator!This full-time (40 hours per week) position offers a competitive compensation and benefits package, with eligibility for a hybrid schedule (one remote day per week) following the introductory period.

Position Summary

Woodfords' Shared Living Program is designed to provide family, home-based services to individual's ages 18+ with intellectual and/or developmental disabilities. An alternative to group home living, adults are matched with provider families who deliver a stable support system and natural family environment that fosters relationship building and community inclusion. The Shared Living Coordinator provides oversight support services to achieve program needs and advocate for consumers' freedom of choice.

Additionally, the Shared Living Coordinator will serve as a liaison between providers, family/guardians, and other service providers to help establish and maintain the HCBS Global Settings Rule and monitor progress toward consumer goals and objectives.

Services will be rendered in a combination of office, home, and community settings throughout Kennebec County, Maine.

Opportunity Details
  • Full-time, 40 hours per week
  • Monday through Friday, business hours
  • $45,760-$49,920 based on experience, education and certifications
  • Hybrid work schedule eligibility - upon completion of the introductory period
Qualifications
  • High school diploma or GED equivalent. Associates and/or Bachelor’s degree in related field preferred
  • Must possess at least one years’ experience working in the field of social services
  • Must have a valid driver’s license, the use of a reliable vehicle and the ability to meet consumer transportation needs in their local community and surrounding areas from their assigned work location
