Travel Occupational Therapist - Home Health

American Traveler is hiring a licensed Occupational Therapist for a home health position serving the Grand Junction, CO area on a Monday-Friday daytime schedule.

Job Details

• Home health setting with patient-centered, in-home visits serving the Grand Junction, CO community,

• One-on-one patient visits in patients' homes,

• Monday-Friday, 8:00 AM-5:00 PM schedule,

• Patient population includes geriatric and medically complex individuals,

• Services include ADL/IADL training, upper extremity strengthening, fine motor coordination, adaptive equipment training, energy conservation, and home safety and fall-prevention strategies,

• EMR:
Homecare Homebase (HCHB); proficiency preferred, willingness to learn accepted,

Job Requirements

• Current, unrestricted Colorado OT license (license required if applicable),

• Graduation from an accredited Occupational Therapy program,

• Valid driver's license and reliable transportation,

• Ability to practice independently in a home-based clinical setting,

• Home health experience preferred,

• Familiarity with Medicare documentation and home health regulations preferred,

• Experience with geriatric and medically complex patient populations preferred,

Additional Information

• Perform OT evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits,

• Develop and implement individualized plans of care based on patient needs, functional goals, and physician orders,

• Educate patients and caregivers on treatment plans, adaptive techniques, home programs, and safety strategies,

• Collaborate with an interdisciplinary team including RNs, PTs, STs, OTAs, and agency leadership,

• Provide clinical oversight of OTAs when applicable and permitted under Colorado law,

• Complete all documentation accurately and timely in HCHB in compliance with Medicare Conditions of Participation and HIPAA requirements,
