Job Summary
Physical Therapist (PT) - Home Health Case Manager - South City Chicago
Location:
Oak Lawn, IL | Job : R141360 |
Schedule:
40 hours per week | Pay Range: $43.30 - $64.95
This position is eligible for up to a $10,000 sign-on bonus and subject to all taxes. You must be working in this same position at time of payout. Current internal team members are not eligible at this time.
What You Will Do- Ability to travel extensively within geographic boundaries in all weather conditions.
- Work rotating weekends and holidays. Adhere to scheduled days and hours based on patient load and commitments. Attend mandatory informational/education/branch meetings.
- Assess, plan, implement and evaluate physical therapy treatment programs established for the home care patient. Facilitate the progression of the patient toward optimal levels of self-care.
- Licensed Physical Therapist in the State of Illinois (or eligible for licensure).
- Completed 1 year of Physical Therapy clinical experience or demonstrated strong spectrum of Physical Therapy clinical affiliation experience.
- Proficient use of home-based therapy equipment and assistive devices (balls, bands, weights, Hoyer lifts, etc.).
- Basic knowledge of computer use (Microsoft Word, email); ability to learn to use computer-based scheduling and documentation system.
- Ability to communicate professionally both verbally and in writing; strong documentation skills.
- Good interpersonal skills, time management and self-organization skills.
- Assures and Improves Quality of Care: Facilitates the patient and family’s right to receive quality, cost-effective care following Advocate’s MVP of Stewardship.
