Medical Social Worker
Healthcare / Nursing Home | Singapore
We are seeking an experienced Medical Social Worker to support residents and families through comprehensive psycho-social care, care transitions, community linkage, and funding support within a nursing home setting.
Key Responsibilities- Conduct pre-admission social assessments and support smooth resident admissions.
- Develop, implement, and review residents’ psycho-social and social wellness care plans.
- Ensure healthy bed occupancy.
- Provide counselling and support to residents and families, including Advance Care Planning (ACP).
- Coordinate discharge planning and linkage to community and home-based services.
- Serve as the primary point of contact for families on non-clinical matters and feedback.
- Participate in multidisciplinary meetings and collaborate with medical, nursing, and allied health teams.
- Support Medifund applications, conversion of private cases to portable subsidy scheme, and administration of residents’ funding schemes.
- Liaise with community organisations and manage external partners supporting resident care.
- Manage and respond to the Home’s enquiries email, including triaging, responding to, and coordinating follow-ups with internal teams.
- Coordinate fire safety drills with the outsourced Fire Safety Manager (FSM).
- Maintain and update Emergency Response Plan (ERP) documentation in consultation with the FSM.
- Recognised Degree in Social Work / Post Graduate Diploma in Social Work
- Registered with Singapore Association of Social Workers.
- At least 5 years related work experience in medical social work, preferably in ILTC sector.
- Strong working knowledge of eldercare services, community care networks, and healthcare funding schemes.
- Strong communication, documentation, and organisational skills.
- Able to work independently and manage multiple stakeholders effectively.
- Strong interpersonal and communication skills; able to manage sensitive situations professionally.
- Calm, structured, and decisive when handling complex or emotionally charged cases.
- Highly organised, with the ability to manage multiple cases, stakeholders, and compliance requirements.
- Proactive, accountable, and comfortable working independently.
- Sound judgement, discretion, and professionalism.
- Comfortable balancing casework with operational and compliance-related responsibilities (e.g. partnerships, fire safety coordination, documentation).
