Dementia Care Advisor/KIT – 18.5 Hours Per Week
We have a fantastic opportunity for Dementia Care Advisor to join our Adult Social Care team at Wokingham Borough Council, on a part time, permanent basis.
As a Dementia Care Advisor, you’ll be the first point of contact for adults, families, and carers who need advice, guidance, and support around dementia. You’ll help people understand their diagnosis, navigate services, and stay as independent as possible.
You’ll work closely with health partners, social care teams, voluntary organisations, and community groups to make sure people receive the right help at the right time. Your work will play a key part in improving wellbeing and helping people live well with dementia across the borough.

- Offering information, advice and emotional support to people with dementia and their carers
- Helping people understand their options, including benefits, community resources and support services
- Creating personalised support plans based on individual needs
- Working with professionals across health and social care to coordinate care
- Promoting awareness of dementia-friendly communities across Wokingham
- Keeping accurate records and using systems to track support and outcomes
- Knowledge of Mental Health Problems in Older People, in particular Dementia
- Strong communication skills and the ability to build trusting relationships
- A person‑centred, compassionate and calm approach
- Good knowledge of local services, or a willingness to learn
- Ability to work independently and manage a varied caseload
- Confidence working with families during challenging or emotional situations
- A commitment to promoting dignity, independence and inclusion
