Contract: Full-Time Locum – 12 Weeks Initially

Join our team as a Social Worker within the Community / Home First Team in the beautiful region of East Lothian. This full-time locum opportunity offers £40 per hour for an initial 12-week period, giving you the chance to make a meaningful impact while working within a supportive and collaborative environment.

• Undertake SDS (Self-Directed Support) assessments for community-based cases involving adults aged 16+, including those experiencing frailty and older adults.
• Carry out community interventions, assessments, and long-term care planning, including care home placements where appropriate.
• Work collaboratively within a multidisciplinary team to prevent unnecessary hospital admissions and support effective discharge planning.
• Travel across East Lothian to complete community visits, using your own vehicle due to the rural nature of the area.
• Contribute to short-term intervention planning and Home First initiatives to promote independence and wellbeing.
• This role is primarily based at John Muir House, Haddington, with some work across East Lothian Community Hospital and occasional visits to the Royal Infirmary of Edinburgh.
