We are pleased to offer an exciting opportunity for an enthusiastic and highly motivated Specialist Physiotherapist to join our evolving Integrated Community Stroke Service at Sherwood Forest Hospitals NHS Foundation Trust.
Originally established in 2009 as a Stroke Early Supported Discharge (ESD) Team, the service expanded in April 2023 to become an Integrated Community Stroke Service. This development has increased access to high-quality, specialist, community-based stroke rehabilitation across Mansfield, Ashfield, Newark and Sherwood, supporting patients following an acute stroke.
The service focuses on enabling a smooth and timely transition from hospital to home, with the aim of optimising recovery, function, and independence during the first six months post‑discharge. Working in the community, you will deliver evidence‑based physiotherapy interventions that have a meaningful impact on patients' daily lives.
You will join a supportive multidisciplinary team with a strong commitment to patient‑centred care, collaboration, and high clinical standards. We are seeking a Specialist Physiotherapist with a genuine interest in stroke rehabilitation, who is motivated by service development and keen to embrace a new challenge. This role offers excellent opportunities for professional development and the chance to contribute to the future direction of community stroke services within the Trust.

Person Specification Knowledge- Excellent Clinical knowledge of Medical and Neurological assessment and treatments
- Ability to prioritise & work under pressure
- Ability to work as a team member as well as alone
- Knowledge of NHS procedures, policies and guidelines
- Management of complex cases
- Presentation Skills
- Degree/Diploma in Physiotherapy, BSc(Hons) or Graduate Diploma in Physiotherapy or equivalent
- HCPC Registration
- Membership of Chartered society of Physiotherapy
- Membership of ACPIN
- Appropriate post graduate courses and development of CPD folder
- Bobath Training Modules
- Clinical Educator
- Post Graduate training / Masters Modules
- Basic Bobath training
- Proven NHS post qualification experience with some at a more senior level
- Recent experience in Health care of the elderly/Stroke
- Supervisory experience
- Experience of audit
Multi‑disciplinary working - Teaching experience
- Community experience

- All appointments are subject to satisfactory medical clearance and the ability to cope with the demands of the role
- Able to cover 8am
-6pm including weekends and bank holidays to meet the needs of the service - Full driving licence and the ability to travel between locations.
- Daily access to own transport
